• Please have your insurance information handy when you call.

• New Patient and Other Disability Forms pertinent to the body area of concern will need to be filled out prior to your first appointment. These forms may be faxed, mailed or emailed. Our staff will direct you as to which forms to print and fill out.

• Bring your paperwork, insurance card, and doctor's prescription to your first appointment.

• Please arrive 15 minutes early in order to prepare and scan the necessary documents.

• We accept most major insurances: Blue Cross, Blue Shield, Medicare, Cigna, United Healthcare, Personal Injury

• We are a "Scent Free Clinic". Please refrain from wearing perfume, scented lotions, cologne, etc.  Thank you for your understanding.
